For our new patients to the practice, we ask that you arrive 15 minutes prior to your scheduled appointment time in order to process your documents. You need to complete the Patient Registration and the Financial Policy forms on our portal . Call our office to register.
Please bring with you the following:
1. Photo ID like a valid driver's license.
2. Current copy of medical insurance card.
3. List of all current medications.
Please call our office for any medical issues during office hours. DIAL 911 FOR A LIFE THREATENING EMERGENCY. After- hours calls should be reserved for urgent matters by dialing (407) 339-5600 or 407-384-1414 and follow the instructions. Under no circumstances will treatment be rendered over the telephone without an office visit.
